 Q. How does it work?

 A. Simply locate a service provider in the network, present your card to the provider at the time of  service and get instant discounts on his  or her services. That’s it! The Formulated Dental Plan is not insurance, so you don’t have to worry about paperwork.  Discounts are  received immediately at  time services are rendered.

 Q.  What if I have insurance?

 A. Even if you have health insurance, the Formulated Dental Plan can save you money on products  and services not usually offered by  insurance, such as orthodontics and cosmetic surgery.

 Q. Is this insurance?

 A. Formulated Dental Plan is not an insurance company, it is a Provider Access Organization that
 offers membership in a discount dental program.  Members can save 20% to 60% on dental
 expenses.  Since the dental plan is not an insurance or a health care organization, all existing
 conditions qualify except orthodontic treatment in progress.  There is no paperwork or forms. 
 
Formulated Dental Plan is not responsible for payment of your dental bills.  You pay the provider
 at the time of service.

 Q.  How much do the vision and chiropractic programs cost?

 A.  The vision and chiropractic programs are now FREE with the dental savings membership.

 Q.  How do I pay my Membership fee?

 A.  The minimum term for Formulated Dental Plan Membership is one (1) year.  
 Membership fees are drafted monthly.  We accept credit cards or electronic checking
 account drafts only.

Q.   Why is there an activation fee?

A.  This goes to the cost of creating and sending out your membership information, personalizing your membership cards and registering  you and your family in all of the different areas of the enormous medical networks so that you may access the savings.  It insures that we  can keep the monthly cost down as low as possible while guaranteeing NO RATE INCREASE FOR LIFETIME.  This one-time fee is  non-  refundable.

 Q.  Who are the Dental, Vision and Chiropractic Care Providers?

 A.  Formulated Dental Plan has contracted with Careington, EyeMed vision care and Healthways to expand the number of
 participating providers across the nation.  All providers are carefully selected and credentials are carefully verified.  We are
 continually adding to our base of providers.

  Q.  What if I want to change dentists?

  A.  We understand that the dentist/patient relationship is highly personal.  You may change dentists   at any time without having to notify  anyone.

 Q.  Is dental work started before joining the program discounted?

 A.  Dental work STARTED before signing up for the dental plan is NOT, even if completed  after  joining.  However, existing dental  problems ARE discounted, when work is started after joining.

 Q.  Are implants discounted under the dental plan?

 A.  Implants and some whitening procedures are not discounted by all participating CAREington  providers.  Check with your participating  provider for further information.

 Q.  How do I find participating providers?

 A.  You can locate participating providers, convenient to you, using our online search (or call us  toll-  free).  Up to 50 names per category  will be displayed online.  Participating providers have met  a  highly selective credentialing procedure based on education, background,  office facilities and  other  requirements.  In order to receive a discount, you MUST use a participating provider.  When  you  call  to make  an appointment, verify that the provider is currently participating with CAREington (dental);  EyeMed Vision (vision); American Whole  Health (chiropractic and  alternative care).

 Q.  Can I cancel my membership?

 A.  We are so sure that you will be satisfied with our program, that we give a 45 day risk-free  money  back guarantee.  No refunds are  given after 45 days.  After the first year of membership,  cancellation  may be made with a 30 day advance written notice and return of  membership cards

 Q.  Which family members can use the plan?

 A.  Family members in your household include your spouse, dependent children and financially  dependent relatives living in your home.
